I knew I forgot something.

So I wasn't as organized as I should've been. When I asked my teachers for recommendation letters, they gave me four copies each. Great, right? Well, I had them fill out the NYU evaluation form, but not the Common App. It completely slipped my mind, and now I have applications to Northwestern and Syracuse due on January 1st. I have the letters but not the evaluation form.
I suppose I could call my teachers and ask them to quickly fill out the forms, but it's Christmas break, and that is extremely rude. I would probably do it as a last resort, though, for Northwestern. Stupid calculus tests...I would've had this done weeks ago. What should I do?

If you can, call the teachers. When you get back to school after the holidays, give them a thank you gift of some home-baked cookies or something thoughtful and small like that. Acknowledge when you call them now, and when you thank them later, that you're really sorry you had to interrupt their vacation.

Oooo, I just found something on the common ap website PDF. It says "A photocopy of this reference form, or another reference you may have prepared on behalf of this student, is acceptable." So can I just basically send the NYU evaluation (maybe photoshop off "New York U", haha)??

yeah

10 charss

It may be possible for some schools to send in the recs after the application deadline, as long as they are reasonably close when reading starts; I am doing that for one of my supplementary schools, and the schools I have contacted (UChicago, MIT, Bates) are okay with it.
