I screwed up on my UC application

In my sophomore year, I received a “D” in both semesters of World History Honors, and a “D” the first semester of Algebra 2 in Junior Year. However, I retook all the classes over the summer and received A’s in all of them. On my UC application, I naively inserted the grades I received in summer (A’s) instead of my D’s. I realized this mistake after I got accepted to UCSC and now I am scared that I will be rescinded because of these mistakes. I have already emailed UCSC admissions and filled out a grade issue form. How likely will I be rescinded? Please help!
(Also on my application I put in World History instead of World History Honors).

You have done all that you can, so it is up to UCSC to determine what they will do with the information. Definitely better that you have contact them now and when your transcripts are due in July. Hopefully being proactive will help.

Best of luck.
