Internship Application Help

Hey guys, I need help on this application for a internship/volunteering opportunity at a clinic.

One part of the application says to put academic/personal achievements. I honestly can't think of any, unless I put stuff from like elementary/middle school. I'm currently a sophmore and although it is not required to put something, I feel like my chances diminish if I don't.

I took an AP test is 8th grade, if that counts?
Ran track last year.
Key Club (Volunteering Club)

I put track and key club in my extracurricular section of the application though.

Will it greatly affect my chances?

Don’t put anything from elementary/middle school. If you don’t have anything, just leave it blank. It’s not that big of a deal, and it’s better than putting something from middle school (or even high school, to be honest, unless it’s something particularly spectacular).

Thanks! There’s an essay portion (250 words max) answering 3 different questions. I think that is going to make or break me. I’m thinking of asking my English teacher for help.
