<p>Don't know about the first part, but on the second: wait for them to mail it, then go to the "Where am I in the process?" part of their website and you can access your application status. It's supposed to take awhile to get it, I hear. :</p>

<p>Might want to try calling admissions, if possible. Best to be sure it's not necessary.</p>
<p>I don't think you need bank statements, just the prelim-app which asks you about your financial situation. Can someone confirm?</p>
