<p>I belive I saw in the common app a teacher evaluation form. Is this THE form that is to be used for 'teacher rec's' for all colleges, or just to colleges that take the common app?</p>
<p>Just for colleges that use the common app. If they don't use the common app, they will either have their own forms or not require teacher recommendations.</p>

<p>Has to be snail mailed. Print it out with your information and then give it to them with pre-addressed, pre-stamped envelopes along with anything else they need. They'll fill it out, make copies, and send it out to the schools.</p>
