Letter of rec from non-academic teacher?

<p>I am asking my private music tutor to write me the second teacher's letter of rec. What format does it have to be? </p>

<p>The form from common app explicitly stated it's for academic teacher only. I don't know if I should use that one.</p>

<p>uhm. i think most schools require 2 academic letter of recommendations (i’m not really sure on that) and the rest are optional. I’m pretty sure the optional ones doesn’t have to follow the common app form but rather could be a letter (literally) instead.</p>
