Letter of Recommendation Form

<p>I am asking a teacher for a letter of recommendation but he cannot submit it online. Instead, I will give him an envelope and he will mail it. He is expecting me to give him some sort of form to fill out for the recommendation. I have looked but i cannot find one on any schools websites. Does anyone have any ideas?</p>

<p>If the schools you are applying to require a rec, they almost certainly provide a form. If not, you can print and use the common app rec form. What schools are on your list?</p>

<p>These are all common app schools. You will be applying through the common app website and you need to create a common app account for yourself to do that. </p>

<p>For the rec form, go to the common app website, download forms section and print the Teacher Evaluation form:

<p>For some odd reason, you can only find the forms if you log out of your CA account. It used to be that you could print them from inside your account, and it would include name/ CA number, etc… I have no idea why they changed that, unless they are really pushing the online recs. </p>

<p>He can usually use the CA form, even if it is not a CA school, BTW.</p>

<p>Ultimately, if you can’t find a form, just have the rec letter include your name, year of birth and HS included. The office that receives it will be able to correctly sort the rec letter – it’s not that big a deal.</p>

<p>It’s not as if they’re going to toss it into a bonfire or a shredder…</p>

<p>Believe it or not, they actually want to read the rec letters because they are looking for reasons to admit you.</p>
