Letter of Recommendation/Teacher Evaluation Form

OK, first off, I'm not an expert with these applications, so please forgive any stupid questions that may follow.

I was wondering if the letter of recommendation and teacher application form are two separate things (on MIT eval form, it says teacher is free to write a letter if he/she wants instead of answering eval questions). Are they interchangeable, as in do we need to submit 2 letters in addition to the 2 eval forms, or are 2 eval forms ok? Secondly, if we do need to submit two letters in addition to the 2 eval forms, do these four things need to be written by 4 separate teachers? I was wondering because 4 teachers multiplied by 4 university applications (thats how many schools I plan to apply to) means 16 teachers to write recommendation letters or eval forms, and I do not know 16 teachers that well. I'm pretty sure that's not the case, as there is no separate section in the application for letters of recommendation.

Thanks

There are 2 separate recommendations, each a single form (Evaluation A for a math or science teacher, Evaluation B for a humanities, social science, or language teacher). Each teacher can fill out the form directly, with the narrative written on the form in answer to the questions asked there, on the back of the relevant Evaluation A or B form (2 pieces of paper total, 1 from each teacher).

If the teachers would prefer to attach a second piece of paper with their narrative on it, that's fine (4 pieces of paper total, 2 from each teacher).

If the teachers would prefer to send just a letter covering the points on the form in their narratives (rather than also filling in the forms), that's acceptable and has been done before (hence 2 pieces of paper total, 1 from each teacher). There is some flexibility there.

There is no need to submit any further letters, unless another letter from someone else would illustrate another part of your character, involvement, or interests that will not be covered elsewhere in your application.

I would note, too, that you don't have to ask different teachers to write recommendations for each school if you don't want to -- teachers will often write a single letter and copy it for each school, then fill in the individual school's evaluation form, so writing 3 letters is not a great deal more work than writing 1 letter.

OK, thats a big relief, thanks!
