<p>DS is going for his annual doctors appointment today so I thought he may as well get his medical forms completed for UA. Am I correct in thinking that the immunization form is the only form that needs to be signed by his physician? The medical history form and pharmacy information look as though they only need the signature of the student. </p>

<p>Your doctor may ask if your son wants to receive the Gardasil vaccine. If that’s something he’d be interested in, may want to call your insurance ahead of time to see if they cover any of the cost. </p>
<p>Note that UA typically requires a TB test, which depending on your area, might not be something his doctor immediately thinks to do. The meningitis and HPV (Gardasil) vaccines are relatively standard for college students nowadays, though check with your insurance to make sure that both are covered (the HPV vaccine series costs around $700 otherwise). While not required by UA, it might be a good idea to begin routine testing for HIV, STDs, and STIs.</p>
<p>I could only find the form for the immunization to get filled out for the DS for next year. Where did you find the other two? He has already uploaded a picture for his action card.</p>
