mmr (measles, mups, rubella) immunization form

<p>i can only find record of when i was 11 and had this shot but i guess you need two to be cleared if your a freshman living in the dorms...</p>

<p>if i can't find the other record when i was a baby, do i need to get another shot? i know i got one after i was born... :-(</p>

<p>S199-My D is coming to UW from Ohio, where the MMR was required in junior high school. I sent the form in verifying that, and got an email saying her shots were NOT up to date. She went in for a MMR booster last month and I mailed off the form and haven't heard anything since. The best we could gather with UW's limited info and communications, was that a booster was needed to attend college in Wash state. This may not be the case, but a booster won't hurt, you can try calling but trust me, it'll be difficult getting anyone on the phone that knows anything. So just get the booster and send off your form if that's possible, bc you cannot even attend orientation and registration without that form completed and shots documented. P.S. I couldn't find D's original baby records, either...but what's so ridiculous is they can't attend school w/o that proof in kindergarten, so why do they need that? They must want a booster. LOL, UW is rather difficult to navigate sometimes!</p>

<p>Hmm...for ALL the shots/immunizations/injections you've ever gotten through your entire life, did you get them all done at the same hospital or clinic? I've been with the same clinic all my life, and I all did was ask the nurse for a record of all of my immunizations and shots, and she was able to show that I did have to MMR's done.</p>

<p>yeah i just found my shot record yesterday from when i was a baby so i don't have to worry about it</p>
