<p>I recently printed out the vaccination forms that I have to send to Tulane by July 15th. Unfortunately, I have no idea why I cannot find record of my vaccinations (MMR, Tetanus, Meningitis). Today, I had to get those vaccinations again. I am still waiting to get MMR (which i am sure i HAD to have had since I cannot attend grade school thru high school without them). But because I am almost 18 (in 6 days), I have to get my first dose of MMR Monday and wait another month to get the second dose. I will not have time to mail these forms in time by July 15th, because I will be out of the country until July 15th. Furthermore, if I get vaccinated on June 15, I still have to wait until July 15 to get the second dose.</p>
<p>Is there any way I can send in the form with my first dose? and then mail the tulane health center an attachment for the second dose? </p>

<p>If you think you had these shots your school records probably have an immunization form in them that you can obtain a copy of. I would at least call or stop in and check that possibility first.</p>
<p>A titer test is much quicker and will check to see if you have the immunities to MMR, thus demonstrating that you’ve had the vaccinations. I looked at the TUlane form and they don’t indicate a spot for titers, so I would call the student health center and ask (504.865.5255)</p>
<p>The Proof of Immunization Compliance form requires a physician, or other health‐care provider, to record the data on immunizations. We would encourage you to get any missing immunizations at home. However, you may complete your immunization requirement during summer orientation sessions or after you arrive on campus. Please give the 4 page Proof of Immunization Compliance form to your physician as it requires a health care practitioner to complete, sign and stamp it.</p>
<p>So, it sounds like you can finish the MMR series once you arrive on campus if they don’t accept the titer.</p>
