Optional Recommendation w/o Birthday and School

Hi everyone I asked one of my employers for an optional recommendation that I'll be submitting with my application. The recommendation made, however, does not mention my date of birth and high school. I am fairly certain I would be the only applicant with my name, so I don't think there should be any confusion with that. It still does say that those are the requirements of an optional recommendation on my early school's (Yale's) website, though.

Would they mind these shortcomings? Is there anything I can do to deal with this, such as specifying that on an envelop or cover sheet? And on that note, are there any specifications for the "cover sheet", if any is even asked for? Are such cover sheets usually letters/notes from the applicant? Or descriptions, not addressed to anyone in particular, but simply specifying the contents?

Thanks for reading!

bump anybody? :(

Yea, just print some information on the bottom left corner of the envelope, such as your common app ID or school+address.

OR if you have a good printer and access to the letter itself, you can put it in your printer and print out your information in the upper corner onto the paper with the letter.
