<p>I was just wondering... when they ask for you to enter information about your clinician/physician for the Hepatitis B Clearance... do they want the information from your current physician or the one who administered the shots? Would my current physician even have the records for that?</p>
<p>well i came from another country… so i took it there… and now i’m here and on the immunization record it says my physician and “transcribed only-not given” on the shots. Do i put this physician? or korean one… which i have no clue who it is.</p>

That is for the physician or clinic who administered the Hep B vaccine.</p>
<p>So hopefully she understood the question clearly and that should be the answer ;)</p>
<p>Hey KCfan, i hope you read this by the end of today, but i called yesterday, and the lady told me that it can be your current physician, as long as they can verify the hep B series info from your immunization card. But I guess it doesn’t make that much of a difference because your previous doc may still have your records on file. I was just worried about mine because my previous doc moved to a diff country!</p>
<p>but i think you should seriously consider the length of time youve been out of the doctors office. You should call them up if they’re still around, and first check if your files are there, and second, tell them to be aware that ucsd will give them a call to verify the hep b shots.</p>
<p>Yikes… good point o.0 I think my previous physician might be almost 60 by now. Thanks for the heads up. I really appreciate it =) I’ll try calling in tomorrow… though hopefully UCSD won’t be too strict if things don’t go so swimmingly xD</p>

<p>I had a question re. filling out the actual form. So… we receive a series of three shots for the immunization. On the site, it states that July 15 was the deadline for the first shot in the series. And on the verification form, we are asked for the dates of the first two shots we receive. However, if we are in the middle of the series and have only received one shot, the second shot’s date will be sometime in the future. The computer still rejects any date that is not “less than the current date.”, or, has already passed. Does anyone know how to deal with this? I’m sorry if my question is confusing.</p>
