On my transcript, it already says all my Hepatitis B crap. Will they take care of this? Or should I do something about it >.>;;</p>

<p>Mm even if your transcript has the Hepatitis B stuff on it already, you still have to send UCSD their own Hep B form. Go to MyApplication and click the link under Hepatitis B; there should be a form that comes up as a .pdf file. Print it and fill it out appropriately and send it to them ASAP.</p>

<p>unfortunately, i'm not able to provide other proof because apparently my health care provider doesn't have my complete immunization records (i switched to a different health care provider 10 years ago) even though i have completed the hep b series. i'm guessing some idiot didn't have the skills to enter data correctly into a computer when my previous health care provider transferred the records. BUT I GUESS, THAT'S LIFE!!!</p>

<p>and also, the deadline is stated as 07/15. does this mean it has to be postmarked or received by that date? sorry, my mind is a bit clouded after some back-and-forth attempts @ resolving my whole immunization records issue.</p>
<p>I don't know, I think postmarked is fine but I would definitely send whatever in as soon as you can. </p>
<p>Again, I don't think proof of the Hepatitis B immunization series can be satisfied by something that shows up on the transcript because UCSD has its own form for proof of Hepatitis B. The high school transcript and proof of immunization go to two different places at UCSD.</p>
<p>Just write in the information for either doctor and when it turns out to be a problem, solve it then-- for right now, just get the form in.</p>

<p>Just the form will do it, no attached proof necessary. I think it's because they know that most high schools and/or middle schools require the Hepatitis B series anyway but they just need you to say on record (via the signature on the form) that you did indeed receive the immunizations.</p>
