A reminder and a question =)

<p>Just want to remind you guys, the deadline for your immunization record is coming up. Here is the info...</p>

<p>Deadline: August 1, 2006. Failure to meet this requirement will restrict your admissions enrollment/registration process.</p>

<p>The California State Legislature mandates that first-time enrollees who are eighteen years of age or younger provide proof of immunization against Hepatitis B prior to enrollment. To assure compliance with the law, see the complete instructions & forms for Hepatitis B.</p>

<p>Where do you send this thing to? (I'm assuming its same address as where out transcripts were sent. Is it?)</p>

<p>The form says to send it to the address located at the top of the form.</p>

<p>does anyone have their hep b form checked off yet?</p>

<p>I turned my in like June 12 but it's not checked off yet.
However, I checked box 1 because I'm going to be 19 in beginning of Sep.
the lady told me that in my case I don't need to submit a Hep.B form. and it wouldn't be checked off for those who are going to be 19 before the enrollment (Sept.21)
I'm kind of confuse????</p>

<p>Haha... my bad! We just have to send the first page right? My doctor didn't fill out the other ones, so I'm assuming the first page is the only paper i need to send them. Is there anything else we need to send?</p>

<p>Yup, just send the first page. The second page is for your doctor to explain to them why an immunization wouldn't be safe for you (That is if you checked box #4).</p>

<p>the address are NOT the same. If you've completely met all the requirements, you mail it directly to admissions office. How ever if your doctor metioned something else saying that you have some problem, you have to send that to the medical center at ucsd, address located on the 2nd page of that paper.</p>

<p>if you have gotten all the immunizations done, you should be fine. my school said i only had part of it done, so when I asked them, they instructed me to go get it completed. I got 2 shots and I was done. Some people got one, some people got three or more. In your case, if ur school sent it out for you, I am pretty positive you've completely the immunization required for entering college. Just to be sure, call the admissions office to see if they've received it or not.</p>

<p>hypoch0ndriac: I believe that you still MUST print out the Hepatitis form and have your doctor sign it to prove that you have finished your immunizations. If your school sent a copy of your immunization card to UCSD, I doubt they'll know who you are because they have to deal with thousands of immunization records. That is why the Hepatitis B form asks for your PID and PAC.</p>

<p>IGotIn is right. But I don't think PAC was required on the form. You still have two weeks so don't panic, but get it done asap. Try calling them first so you don't have to waste time going back to doctor getting that thing signed and then going to postal office mailing it.</p>

<p>just complete the 1st page with a MD/doctor stamp and send that form to the address below....that's it!</p>
