Questions on Health Forms. Please help!!!

<p>Hi people I need your help!</p>

<p>I have not taken some of the required immunizations stated.</p>

<p>Must I fill in the date(s) on the online form for the immunizations part before I can print the health forms out in PDF? Or is it okay if I write in ink on the already printed forms after I take the jab?</p>

<p>Plus, must I take the required immunizations before I send the health forms by mail before June 10? Or can I take the required immunizations after that date?</p>

<p>you can call Gannett Health Service at 607-255-5155 for more info… I think this is how it works. A) Fill out the online Q with the date/type of immz. B) Print the forms and have your Doctor validate/Sign the forms. C) Send all sign forms and ANY NEW COPY of IMMUNIZATION.</p>
