Questions regarding Insurance Verif Form deadline extention & medical insurance

Hi moms and current students of UB, would you spare some of your time to help me out?
I've only found out about the requirement to have medical insurance for college after checking up my bills on Hub. I filled out the wavier on 8/4, and I received a request for me to fill out a form that require the signature of the insurance representative within 2 weeks. Problem is, I have applied for Straight Medicaid(which allow me to move the primary care location to buffalo) and I'm still waiting for the actually card to arrive in about 4-6 weeks, and no representative will sign the form until then. I wrote a letter to the university explaining my situation, what are the chance that they'll extend the deadline? or rather who should I contact to increase my chance?

I'm afraid my request will not come through, therefore I decided to also apply to an insurance plan that covers over buffalo area, but I don't know any insurance company that covers buffalo area and fits the university's list of requirement. Will anybody please suggest any medical insurance company that they or their children use for UB?(Preferably a company that I can find representative in NYC or somewhere nearby.) I would really appreciate your help, thank you.

I don’t know what Straight Medicaid is, but Fidelis Care is in Brooklyn, participates in most of the NYS plans, and has very good coverage in Buffalo (888-343-3537). Who is your carrier now? Can you call their member services line and obtain a fax number from them? I suggest you contact the Director of Student Medical Insurance for SubBoard I for help. Her name is Catherine Engelhardt-Ellis and her contact info can be found on this page:

Hope that helps!

Your best bet is to call SBI and ask for their assistance.

thanks sk8rmom and jojoyohan for the help, but what exactly does SBI do? will they be able to give me a reply from UB’s prospective?

It is actually SBI that processes student medical insurance waivers so, yes, they are acting for UB. If you search the UB site for student insurance, it will lead you to SBI’s info.
