Readmission - Listing schools attended

Basically, I was a transfer student in UCLA back in 2012. I attended my first Fall quarter and left the college (or leave of absence) due to financial/ personal problems.

I am planning to submit the readmission application for next year Fall 2016 and had some questions. Btw, I left school at good academic standing.

  1. It stated to list all the institutions that I've attended after leaving UCLA. If I don't list all the schools, will they be able to find out? (The application said, incomplete listing might cancel the readmission, but how will they be able to find it out?)
  2. I have been working full time and have attended few community colleges here and there to complete my pre-reqs, however, I have received three W's and two A's so far. Now, the W classes, I am gonna try to make it up on coming Spring semester with a letter grade.. But one thing I'm concerned about is that one of the colleges that I've attended, it shows one W and one letter grade and honestly, I do not want to disclose this institution if it will negatively affect my readmission (these two classes were part of CC pre reqs for the class I NEEDED to take - ended up not taking the main class). I want to reduce showing W's as much as possible. But, if I happen to disclose all the institutions, will the W's affect my readmission?

Seems like there’s not many threads regards to readmission.
Please help me out!

By querying the National Student Clearinghouse. If you “forget” to list a college they will catch you and cancel your admission. Do a search here, its happened to people before.

sigh…okay. I guess I should list all the colleges I’ve attended.

Have you heard anything about the withdrawal part?
Will it affect the readmission?
I never had a withdrawal before I transferred and was at good academic standing before I left UCLA.
