<p>I'm applying to UMass-Amherst but I can't find the teacher reccomendation form on their website. Any idea where it is or do the teachers have to make up their own?</p>

<p>umass uses the common app, if you use the commonapp to apply to umass, then you can use the commonapp teacher evaluation forms on the commonapp website</p>

<p>Just have your school include the letters of recommendation with the transcript package they send to the school. There are no special forms for the letter of recommendation. I spoke to UMASS admissions about this a few months back.</p>

<p>They want separate rec letters (they don't have their own form) but they do accept the common app for the actual application part.</p>
