<p>I keep reading that there is a set of forms that needs to be given to one's teachers in order for them to write you a recomendation letter. The thing is, I have no clue what forms everyone is talking about. Are these forms specific to each school, or is it just the Common App Printed out? Apparently, these forms require teacher information to be included. Generally, I'm just confused. Can somebody clarify this for me?

This link has all the forms. The one that says Teacher Evaluation is the one that the teacher fills out.</p>
<p>The Common App has a Teacher Evaluation form. If you sign up for the Common App online, the forms will be taken care of when you invite teachers in the School Forms section.</p>
<p>In my case, I just have to ask the teacher if he/she will write a recommendation for me, and they give a completed one to my counselor who uploads it to the commonapp or whatever, and I just check some sort of box when I submit the commonapp to schools, i think. I don’t know if others schools do it like that or not though.</p>
