Recommendation from non-teacher/counselor

<p>I am getting a recommendation from a professor I worked with over the summer.</p>

<p>Does she need to fill out any forms like the ones that are required for teachers/counselors? Or can she just directly mail in her letter in an envelope with my name/address etc. on it.</p>

<p>I might ask her for 5 or 6 copies of a recommendation typed on letterhead that you can then use for all your applications. Unless the college(s) you’re applying to specifically asks for a form to be included with the letter, you can simply include it with your application (or send it separately if applying online.)
