Recommendation help

<p>I have already asked for two recommendations from academic teachers: biology and history, and I have used the Common App Teacher Evaluation form for both.
However I also want to ask for a recommendation from my mentor at a laboratory I have been working at, and so far, I have been doing my application (ED) online. Since there is no formal form for an outside person, how would I get the recommendation from my mentor to the school? Is there a separate form or do I just ask the mentor to directly send a letter to the school?</p>

<p>Also, I am applying early to UPenn, and I also am wondering what happens if my mentor and/or my teachers send in the recommendations before I finish applying online? Do they toss the letters out or do they make a new file?</p>

<p>I asked a question like this a bit ago, and I found out that when the first part of your application arrives (be it a rec or your test scores) a file is made for you for future parts of the app. So it’s okay if your letters arrive early. I’m not sure about Penn, but for some schools there is no specific form for an extra letter, they just have the recommender send the letter through the regular mail with it clearly marked as “Supplementary Letter of Recommendation for Han607” or something like that.</p>
