I need to make a resume by tomorrow for my GC, but I'm not really sure what I should include or leave out. Also, if anyone has a good format to use, let me know.

Is this resume just going to be used internally at the school to help the guidance counselor write your recommendation? If so, the format really isn’t important. Just make sections and list items in each one: School Activities (list the school extracurriculars you are involved in–band, sports, clubs–and what your involvement entails–whether you have a leadership role, etc.); Other Activities (non-school related extracurriculars, like a job or volunteer work); Honors and Awards (if any); Hobbies and Interests (playing a musical instrument, designing websites, reading). That should do it. Information about your academic life should be something he/she already has, so you need to provide the rest of your story.

This is to assist my GC and teachers when they’re writing recs.

Since the teachers probably don’t have access to all your academic records, I’d put your freshman through junior year cumulative GPA (weighted and unweighted) at the top, along with a list of your current classes, so they can put your credentials and accomplishments in context with the other students they are writing recs for. Just make sure it’s easy to follow. Use bullet points and short phrases.

Your resume can also include the following items:

Name, address, phone number, and e-mail address

Catagories can include the following is applicable:

Education (current school, grade level, gpa, anticipated graduation)
Work Experience
Internship History
Enrichment Programs/Summer Programs
Community Involvement
Leadership & Activities
Awards

Hope this helps.
