SAP & Maximum Time Frame Appeal

I have been out of school for over a year. My last 3 semesters I completely withdrew from all of my classes and was granted a medical withdraw for problems with depression/bipolar disorder. I have been working and holding down a job since I left school and I am now wanting to go back to school to finish my degree. I have only completed 60 hours out of 170 hours attempted (<50%) but still have a 3.15 gpa My plan is to take 12 hours that I can take at my local junior college before transferring to the State university in May and taking classes over the Summer, Fall, and Spring and graduating in May 2016. I have been seeing a doctor and have learned how to deal with my bipolar disorder so I finally feel ready to complete school while continuing to get treatment and counseling for bipolar disorder. Does anyone know what I need to do to have my appeal approved? Have a screwed myself? I've heard/read that maximum time frame is hard to appeal.

Thanks for your help!

You need to contact your college. The SAP appeal policies vary by school.
