When is toward the end of the preceding semester. The where and how varies by college.

You’ll be notified in some way (probably from your school’s advising center via email) regarding the date and time you can begin registering for classes. Depending on how many credits you have, your class standing, your status as a student-athlete/honors student/etc, your registration window will vary relative to the University at large.
Typically you meet with an advisor and figure out what requirements and prerequisites need to be completed in what order and then you’ll be left to choose which specific classes/professors/times you want to fulfill such requirements.

It varies by school. I registered for my first-semester classes in July before school started, and obviously I didn’t have to physically meet with anyone. For every other semester, you can register starting in April. The exact date depends on when you plan to graduate, which isn’t necessarily the same as your class standing. (I gained senior standing this year, but I had to wait until the last registration date because I don’t want to graduate until 2018.) The online system won’t let you register until your adviser logs in and gives you permission. In theory you’re supposed to have a discussion about requirements and prerequisites, but I’ve just gone to his office and asked him to take the holds off my account.
Right, mine’s similar to preamble’s system. You meet with your advisor(s) (in most cases, this is optional but highly recommended). Then, in October you log into your online portal and look up your ‘time ticket’, which is based on your number of credit hours you have, including those you’re enrolled in, and if you’re in the honors college, which gets priority. Then, at the given window of time, you log in and add/drop classes for the Spring semester. It all happens again in March for Fall.
You can often also look up how to register for courses on your school’s website. You might want to jot down any pertinent deadlines on your calendar in case you accidentally miss an email or something.
