Scholarship Acknowledgement Form

<p>Hi there,</p>

<p>I recently got the Leadership scholarship from USC and to formally accept it, I was supposed to:</p>

<p>"2. Sign the Scholarship Acknowledgement Form and return it to the Undergraduate Scholarship Committee
by May 1, 2008."</p>

<p>But the said form was nowhere to be found. The only contents of the envelope they mailed me was the letter and a small brochure detailing the instructions. Did anyone get the form? And any suggestions on how I could get it too?</p>

Other posters have said their acknowledgement form was missing as well. It should not be a problem. They will probably mail it in a day or two as the calls come in. There is plenty of time before May 1st. If you have not received the form by then, write a short letter saying you accept the scholarship and attach a photocopy of the award letter.</p>

<p>I’m wondering if all the non-Presidential/Trustee scholarships have been mailed? I hope not!!!</p>

<p>My S still hasn’t received any scholarship info. He interviewed in February. Wait a few more days…</p>
