Stanford transfer question

For Stanford transfer applicants, which documents need to be submitted electronically and which need to be submitted by mail?
For freshman applicants it seems that everything can be submitted online, including teacher evaluations, but for transfer applicants it's much more arbitrary. As far as I can tell, the Common app and Stanford supplement need to be submitted online but most of the reports and evaluations must be mailed (they have those little mailing labels).
Should I send all of my documents together in one package or does each letter and report need to be submitted individually by the person who wrote it?

One more question: does anyone know whether Canadian students have to submit the International Supplement? The curriculum of a Canadian high school doesn't seem to be much different than that of a US high school.
Thanks.
