<p>Thought it'd be good to have something for future applicants to refer to. I know I was freaking out when I was filling out mine; there's really little information on it.</p>
<p>I got the Borderline one, so I can provide the questions from that questionnaire. But the other two (talent and disability), I don't. If you received one of those two, could you provide the questions from there?</p>

<p>Do you believe your grades and test scores accurately reflect your academic ability and preparation? Please explain.
Talked about how my GPA might be low, but that's because my school's tough and I trust their judgment in my grades. Said I'm fine with my SAT score because I knew I did my best with what I have.</p>
<p>Describe in greater detail any special talent, skill or ability you included in your list of extracurricular activities or mentioned in your personal statement for the UC Application and how the pursuit of this activity may or may not have affected your academic achievement thus far. (Art, music, dance, debate, athletics, volunteer or community service, etc.)
Talked about how I've been doing music for a long time and liked it because I liked the sense of achievement it gives me, but also how I didn't really let it interfere with my academics, in effect learning how to prioritize and separate time accordingly for hobby and school.</p>
<p>Are there any personal circumstances such as employment, family responsibilities or illness that have significantly hampered your ability to achieve academically thus far? How do you feel these circumstances have prepared you to overcome any personal challenges that might occur while at UCSD?
I talked about how my family's finances limited my academic opportunities, or at least I thought so until I started the college application process and found out I'm eligible for all kinds of fin aid, in effect making me realize if I had just tried harder, I really could've done a lot of things.</p>
<p>Comments: I only learned how to write good college application essays (or good essays in general, really) after application season was over, so I tried to go all out on this one. I'm very happy that it all paid off.</p>
<p>And worthy of note: I did think my chances at UCLA was zilch since I got a borderline supplemental from UCSD and no sup at all from UCLA. But as it turns out, I got accepted to UCLA without any sup! So if that's the case for any future applicants, still have hope!</p>
