<p>Yes, and cross out the word "teacher" and write "supplemental." But aren't you past the deadline???</p>

<p>I sent in a supplemental rec, but it was just a letter - no additional form enclosed. The tone of the letter made it obvious that it was a supplemental rec, but other than that, no proper identification. Of course, I don't know if it was ever even read, but that's what I did.</p>

<p>Oh, in that case, it might be good to attach it to your "update" letter. Like, "Hey Yale, here's what I've been up to, your still my first choice, I love you, my grades are currently such and such, and I've attached an additional letter of recommendation from my ___ teacher/mentor/coach..."</p>

<p>But if they're not your first choice, don't say that, obviously! If you majorly love Yale, you can say "If accepted RD I will attend."</p>

<p>I doubt i'm going to ask my teacher to give me the letter to mail. He will probably mail it directly...</p>
