Supplementary Rec

<p>I have searched online and asked countless people, yet I still am unsure on how I am supposed to send supplementary letters of recommendations. How would the college know it's for you, if let's say a research mentor is writing it who is not affiliated with your secondary school.</p>

<p>Is there a specific form? [I know some schools like MIT supposedly have them, but all the schools I'm applying to are Common APP and do not specify].</p>

<p>One of my friends suggested I just submit the Teacher Evaluation form in the envelope, but I'm pretty sure that is wrong.</p>

<p>Is there some sort of header that goes on top of the letter, a form, how does it work exactly?</p>

<p>If your colleges don’t give specific instructions, here is my advice: </p>

<p>There is no form, they simply write a letter, on their company/school/etc. letterhead if possible. </p>

<p>In order to match it up with your application, they should include the following type of information at the top of the letter:</p>

<p>Label the letter as ‘Supplemental’, so it doesn’t get mixed up with your required LORs.
Your full name as it appears on your application.
An applicant number, if a school has given you one.
Name and location of your High School.
Your date of birth.</p>
