<p>Here’s the excerpt from the email:
To access the TB Screening Questionnaire, go to <a href=“https://shs.ucsd.edu%5B/url%5D”>https://shs.ucsd.edu</a> . You will need to sign on using your UCSD Personal ID number (PID) and your password. This will take you to the Student Health Service Online Services page. From there go to Forms in the left column, then scroll down to the Tuberculosis Screening Questionnaire.</p>
<p>There are also three other forms you have to fill out: Health History Form, Notice of Privacy Practices, and Treatment Agreement.</p>

BTW, I don’t think it hurts to fill out the forms even if you have your own health insurance. The SHS just wants a record of your medical history and in order to receive treatment at SHS, you have to fill out those forms.</p>
<p>They give you until the end of fall quarter to complete the TB test. Don’t panic guys.</p>

<p>I think you get a confirmation when SHS receives your TB form. I faxed mine in and a day or two later, a health staff from SHS sent me a message on the SHS website.</p>

<p>I called them up and a very generous person spent time to contact many people to make sure my stuff was sent in. But other than that, you don’t get a confirmation.</p>

<p>Talked to a heath advisor @ SD and she said you can get the Tb test done on campus, just check into student health.</p>

<p>Mmm, it wasn’t that bad actually. It was like a long pinch. It started off okay, and then it hurt a little when she put the tb stuff in my arm. But, overall, it was okay! Not too different from a shot :)</p>
