TB testing page!

so on mozilla firefox, it doesn't work! i tried internet explorer and the page to print out won't work either! is it just me?

oh and other than TB testing, do we have to do the other health related forms?

You need an up to date adobe acrobat read or acrobat to see the .pdf of the form. Adobe acrobat reader is for free on adobe’s website.

reinstall adobe reader, because you might have a pirated acrobat.

f our lives when it comes to health stuff.

Did you guys also do all the other forms?

^ how many forms are there?

tb, hepb, alcohol, and optional health waiver is all isnt it?

^yeah.
is there deadline for this? its around September right?

TB!!! On my tritonlink page, the checklist thing. There is the hepb section, and I filled in the info and it’s checked off, but I didn’t see anything about tb. (I barely even know what tb is…><")

you should have gotten an email about the tb thing. I don’t know all the details, but I know if you have TB, you cough up blood and stuff. Its the disease that killed pretty much every woman in Edgar Allen Poe’s life.

I didn’t get any email about TB! :open_mouth: Panic mode~~ Was everyone supposed to get the TB email???

i dont think everyone got the email. they probably looked at the uc app and determined who should get tested from that. from the other thread it seems like if you’ve been out of the country you need to be tested.

@101921 if you want to see the email/info for the tb email check here [Student Health Service - Tuberculosis Information](http://studenthealth.ucsd.edu/tb.shtml) at the top it shows a copy of the email they sent out

The email was not sent to your acms email or your XXXX@ucsd.edu email. It was sent to your email that you used with UCAP.

Ironically I was sent it twice the same day… =P

I’m pretty sure it was sent to everybody. Check your inbox again, the Spam box, and the trash box in case you already deleted it without realizing. If you have multiple addresses, check them all I guess.

Ok, so I checked out the website and complete the questionaire. According to the results (didn’t go out of US, never had a history of TB, etc) I don’t have to do anything else in regards to TB, right? Oh but there were some other things that I supposedly have to complete, such as health history form and some other health related stuff, that I had to do on the same website. They had a checklist of like 4 items. Good thing you guys showed me the website. Thanks! [:

But I checked everything and ucsd never showed me the site… ><" I don’t know how I would have known about this.

with the TB form theres also like 3 other forms:

Health History
Treatment Agreement
Notice of Privacy Practices

The treatment agreement seems a little weird so I didn’t finish it yet.

DO WE HAVE TO DO ALL OF THEM?

i dont think you have to do all of them if you have your own insurance (like if you apply for a SHIP waiver).

for 3 forms other than the TB test, did you guy all agreed to it and proceeded? i don’t want to read all the agreement pages…lol what happened after you guys proceeded?

I just consented on everything. After you consent, it’ll be checked off.

regardless of people having health insurance, they should do these forms and agree to it right?

Yes of course. UCSD needs to know these things especially if you are going to dorm/ practically live there.

I called them and they said only the TB test is mandatory if you’ve been out of country and nothing else is if you have insurance. You need to agree with the other stuff if you ever want to use UCSD health facilities. Like band-aides. I just got some shot in my forearm today at Kaiser. I guess immunising is illegal in this country
