Teacher Evaluation vs. Recommendation

This may have been asked by someone else already but I couldn't find anything on the search.

Is the Teacher Evaluation in the Common App and others the same as a letter of recommendation or are they separate entities? If not, should I ask the teachers to just attach the letter to the Evaluation? (I'm assuming it is supposed to be photocopied and given to each teacher with the envelope.) Sorry, I know it's silly, I'm just confused.

Also, what should I do if my guidance counselor hasn't known me very long and doesn't know me very well?

They are the same thing.

Make a point to at least talk to him/her a few times before you apply.

Many guidance counselors don't know a majority of their students due to the sheer volume of graduating class size. If you want a stellar recommendation from them, then it would be a good idea to reveal your personality/character through meeting and talking. If you aren't too concerned about it, then let your counselor do the work -- they are probably familiar with your transcript and know of you through teachers and other school officials.
