Is it okay if my teachers do not fill out the recommendation form but attach a letter to the form and send it in?

It looks like they can attach a letter, but it is in addition to the regular rec form. So they still have to fill out all the questions on the form.

Many teachers (especially at public schools) do not fill out the form, and just attach their letter to it. Just give the forms and the envelops to your teachers, and let them decide what to do with them.

i don't think it's absolutely necessary to fill up all the answers to the questions. you can just ask them to fill up the check boxes and submit the letter.

Can anyone confirm that I can just have them check the boxes and then attach the letter along with the form.

Only the admissions office can "officially" confirm this.

I can tell you that there are students who had it done this way that were admited in previous years.
