Teacher's recommedation letter

Here are what were said on the Stanford applicatin instructions:

"Teacher Evaluations

Qualitative accounts of your academic abilities are important to us, and your high school teachers are usually our best sources of this information. We require two letters of recommendation from teachers who have taught you in grades 11 and/or 12 in two different subject areas from among the following: English, mathematics, science, foreign language and history/social studies. The best piece of advice we can give, as you choose teacher recommenders, is to choose people who like you and who will share unique examples of your academic strengths and personal qualities. We will accept a maximum of two teacher references.

Your teacher will need to mail the evaluation form directly to our office, please provide your teacher with the appropriate mailing labels."

It is not clear from these instructions whether the Teacher's Evaluation forms from the Common Application are needed. Can an applicant just ask the teacher to mail a recommendation letter without the form? I'd appreciate any clarification.

Sorry. The last sentence of the instructions did mention "evaluation form"

You have to give your teacher the form and a stamped addressed envelope. The teacher may choose to just attach his/her letter to the form (many teachers do it because they have to send multiple letters for each student, and don't have time to fill out those forms over and over), but the form has to be signed and submitted.

Thanks, nngmm, for the clarification.
