Transferring from Michigan State to University of Michigan as a First-Year

<p>Alright, so, I'm a first-year sophomore at MSU and, due to some unique circumstances, I'm looking to get into U of M for Spring 2012. I've been filling out the Common App, but I have a few questions:</p>

<ol>
<li>The most obvious question is regarding my chances of getting in. I had a 3.7 in High School but it was heavily weighted by all the AP classes I took. Due to a lack of transportation and time, I only did one extracurricular. I got a 31 on my ACT. </li>
<li>I'm pretty sure the address for MSU that's listed on the app is wrong. How do I find the correct address?</li>
<li>The school forms confuse me a lot. I don't understand what I'm supposed to do with these.
I have four forms to get filled out: The college officials report, instructor evaluation, school report, and mid-term report. I obviously don't have an established record or rapport here yet, so I don't know how I would possibly get these filled out.</li>
</ol>

<p>I would really appreciate some help with this, as I would very much like to get into U of M as soon as I can.</p>

<p>Also, I asked on the U of M transfer webpage about instructor evaluations and I was told that they were not required even though they were on the school forms section of the CommonApp. Does that go for other school forms as well?</p>
