<p>It’s been a few years since I filled out the common app, but I believe you are supposed to submit two teacher recommendations and a counselor recommendation.</p>
<p>When I did it, you had the option to get one rec from the counselor and one from a teacher no problem… what are you having problems with exactly?</p>
<p>The forms are separate for teachers and counselors. Both are given forms with a few questions and a ranking scale. They attach their rec letter to this form.</p>
