I found the teacher evaluation sheet for USC, and filled out my part of it. Then I sent it to my teacher, who will mail in a letter of recommendation. But then I went back to USConnect and went to the admission documents link. It takes me to this document management page, and only has the forms for faxing in documents (it has this cover sheet and back sheet for faxing it in).

However, my teachers as well as my counselors prefer to mail in the sheets. Is it still fine for me to mail it in? I used to be able to find the school forms (like the mid year report and secondary school report) online at the USConnect website, but not anymore. Has USC changed to a fax-in only school?

I can not find the print-out documents anymore on USConnect. Is this supposed to happen or not? The "Admissions Documents" link just takes me to the wierd document management page, and that only has instructions for fax-in or electronic upload.

Hi funnyman4!

Post #8 here http://talk.collegeconfidential.com/university-southern-california/952301-new-applicant-toolkit.html?highlight=applicants+toolkit+2011 has a link to the pdf version of the form you need - Form 4: Counselor/Teacher Report. Your counselor can use it for the secondary school report and your teacher can use it for a teacher recommendation. Print out two (or more) copies.

It is perfectly fine to mail in those forms. In fact, for recommendations, that is still USC’s preferred method of receipt.

Your mid-year report will consist of entering your grades online and mailing or faxing a copy of your report card - easy! No form is required.

Good luck!

alamemom, I agree w/what you stated above :)

haha! Thanks spidey! Have you decided on a major?

OHHH I see. I was just expecting a specific “Mid-Year Report” form, since the common application does that. Thanks! It makes sense. However, I did see a “transcript request form” from USC. Do I send this in twice? From my understanding I have to send in a transcript at the time of application AND once after first semester ends (the mid-year transcript).

Thanks for answering my previous question and questions!

Yes, there is a Transcript Request Form (Form 7), but you don’t have to use it. USC provides it as a convenience if you need it. It is available in post #8 of the New Applicant Toolkit thread I linked. You can use that to sign and submit to your registrar to ask them to send an official transcript to USC, but you don’t have to.

You can use you school’s request form, USC’s request form, you can mail the sealed transcript with your recommendation(s), or mail it by itself, or your school can mail it or submit it electronically. As long as an official transcript makes its way to USC, they are very flexible about how you get it there.

Your **midyear report **is as I described it in post #2: You enter your midyear grades online and then mail or fax a copy of your report card. No transcript is required.

Ok and just to check if I have this all straight:

The secondary school report is a combination of the teacher/counselor recommendation form AND the transcript request form. This is the transcript that will make its way to USC at the time of application.

Then for the midyear report (which I hear that it is not needed unless requested by USC) I will follow what you say in post #2 and enter the midyear grades online at https://camel2.usc.edu/AdmSeniorYear/ and then mail or fax them a photocopy of the report card. But this is only if they specifically request it?

From what I am getting, I will not need my school registrar or counselor to do anything for the mid year report-just send in the grades myself, is that correct? That’s very nice of USC, since many other schools require ANOTHER transcript to be sent–the so called “mid year” transcript. So that effectively doubles the cost for transcripts for each school since two need to be sent…

Thanks for the help, alamemom! I got back the confirmation for reciept of the Part I application in a nice letter that they sent me.

You have everything correct except for
The only applicants who don’t have to report midyear grades are those who are accepted in late January and early February for scholarship consideration. EVERYONE else has to submit their midyear grades online and mail or FAX a copy of their report card. As you say, no transcript is required (yay!).

You may have heard that info from a transfer applicant - they only have to submit mid year grades if they are specifically requested while freshman applicants all have to report them.

I see! Thanks for answering, even this late. I think I’ve finally got it, thanks to you!

You’re welcome!

This has nothing to do with the questions in this thread, but I wrote something wrong in my post #8 - the transfers get SPRING grade requests, not mid year. (See? I am even nitpicky with my own posts!!!)

alamemom, nop not yet…still undecided
still need some more suggestions…

I recently entered my mid year grades online on the USC website. However, i made an error in entering few of the grades on the online form and they are contrary to the ones in my report card. I wanna know the procedure to edit the grades online so as to avoid any complications in my admission process.

varungk92 I made a mistake on my mid yr report too but all i did was resubmit it with the correction. Idk if that’s gonna work. That’s why m trying to log in to USConnect to check my document. Is USConnect down or something cause i can’t log in?

Yes, the whole computer system was down most of the day. The message on USC’s website said that the server overheated this morning. They thought it would be back up by this afternoon, but it’s still not working.
