<p>I'm planning to use the common application to apply to Caltech and Stanford. I have a question regarding the forms that I printed out. For the midyear and final report form, when exactly does my counselor have to mail those forms? I've been wondering since I gave her all the forms yesterday, but she was asking when the "Midyear Report" and "Final Report" needed to be submitted. If anyone can tell me, that would be great! (I'm really not sure when the deadline to submit any of the forms are.)</p>
<p>I'm also a bit confused on my USC application since I'm submitting it online. I don't know how or when the supplementary forms should be submitted (recommendations and transcripts). If anyone could elaborate on this, again it would be greatly appreciated. Thanks!</p>
<p>For USC (assuming Uni of So. Cal): Submit Part 1 online anytime ( includes just basic info). You will recieve a USC ID #. by email or by mail. I think D received both.
Download the Secondary School Report and give paper to counselor. Counselor will fill it out, add a rec and enclose the transcript and school profile, and send it by mail. Make sure you add the ID # in the top box where indicated. (a stick-on bar code is not necessay).
Download Teacher Evaluation and give a copy to one or two teachers. They also will send it by mail after filling out and adding a rec.
These forms will have your name printed on the bottom (automatic after you've submitted Part 1)
Make sure you sign the waivers, a box on the forms (more credible if you agree not to view the recommendations). Give the counselor and teachers self addressed stamped envelopes, with your school as return address (not your own).
Deadline for these documents (as well as rest of app) is Dec 10 if you want to be considered for scholarships, Jan 10 (I believe) if not. Counselor and teacher forms can be sent anytime before deadline, do not have to be sent before or after your own online Parts.(other than Part 1). USC will keep track.
Applying online with the rest of the application while those above forms are sent by mail is fine.
Mid year report paper is given to counselor after you recieve your first semester grades, and then he/she sends that in later.</p>
<p>uhh.. you're mixing the common app and USC app together, I'm sorta confused now. The USC form does not have my name on the bottom even though I did send the part 1, however, the common app does have my name at the bottom. I think I got enough information, but I still don't know when the "Final Report" is to be submitted by my counselor.</p>
<p>yes you are right, no name on bottom...USC forms identified by ID # at top. Maybe you should call USC to find out about Final Report, or email the regional counselor.</p>
