<p>In order to apply for a F-1 visa you need an I-20 which is issued to you by your college after you accept their offer of admission. That means: you cannot apply for a visa before you get accepted.</p>

<p>I don't think it is. I've personally heard of no one whose visa has been denied and of only one case who had to undergo an extensive background check because he shared the same name with a person who is "extremely unqualified" for a visa (apparently this person had to defer his enrollment for a year because the process took a while).</p>
<p>I think they would only deny you a visa if they expected you to do something illegal in the States (e.g. if you have a criminal record, plan not to leave the country after graduation, or if you don't have enough money to finance your education so one might expect you to raise money through illegal means)</p>
