<p>I don’t think they have a specific form. The best way to contact the professor would be through e-mail, which you can find in the school’s online directory. Good luck though with the college process!</p>
<p>Sorry, you misunderstood. I live in New York. This professor was just at a local college, CW Post, not W&M. Should I just get a general recommendation, or is there a specific recommendation form? Much obliged.</p>
<p>Well, there are forms on the CommonApp for recommendations, so once that comes out, try to send him/her a recommendation request with the previously mentioned form. Sorry for the misunderstanding.</p>

<p>Teacher recommendations do not require any particular form for W&M. You’re welcome to send the professor the Common Application’s Teacher Evaluation form when it becomes available August 1 (the professor can also download the form from our web site as of August 1) or the professor can simply write a recommendation letter and send it to us without any particular form.</p>

<p>Yeah, you can send in a general rec, but I personally think it appears better if s/he fills out the rec form on the CommonApp site as well.</p>

<p>Also, you might want to treat this recommendation as a supplemental one, and ask a junior or senior year teacher to fill out the Common App form.</p>

<p>alternatively, you can get the professor’s email address, and then figure out how to get the common app form to him/her when it becomes available.</p>
<p>a general recommendation is just fine. We have no preference for whether or not a Common App form is included. If however you do not include the Common App form, make sure the letter has your full name and some identifying information (birthday or address) so we can make sure to match it with your other materials.</p>
