<p>Once you send in your Part II app online, there will be forms to download on the bottom left of your application page. They’ll be personalized with fax cover and end sheets, as well as your own barcode. Just give them to your teacher and there are instructions on how to fax them. This is the best way to do them since you’ll get instant e-mail confirmation when they arrive, and they arrive to USC immediately as well. (This is what I did)</p>
<p>Otherwise, you can just print out the forms without the fax sheets and send them here:</p>

<p>I think usually the teacher is supposed to send everything in, so you could just print out your USC Teacher or Counselor report sheet and give it to her with a self-addressed stamped envelope.</p>
