Who will apply/applied for the Health Insurance Plan Waiver?

<p>I'm going to apply for it because I already have one...(I live in NY)
Is anyone going to apply for it? and did anyone apply for it in the past? Were your waivers accepted?</p>

<p>My sons both have waviers. There is a form you have to fill out and then include copies of the front and back of you insurance card.</p>

<p>I just relaized that you are from NY. Make sure your insurance coverage will cover you while you are in CA.</p>

<p>"Criteria for Waiver
To waive the USC Student Health Insurance Plan, your other coverage must:</p>

<p>Must provide at least $500,000 in lifetime aggregate coverage (no per incident maximums)</p>

<p>Policy must have no major exclusions – pharmacy coverage, mental health coverage and maternity coverage (female students) are required</p>

<p>Must have effective dates covering the entire semester for which the waiver is requested (verification may be requested by USC student health insurance office)</p>

<p>Must have a policy year deductible of $2,500 or less</p>

<p>Must pay for at least 70% of all covered expenses to providers in the Los Angeles area. Emergency coverage only in Los Angeles is not accepted for waiver</p>

<p>Maximum out of pocket expense on the policy (deductible plus out of pocket maximum) must not exceed $7,500</p>

<p>Identification card or policy with student name and/or policy information allowing USC to confirm eligibility must be provided</p>

<p>Plan documents must be in English with currency amounts converted to U.S. dollars and an insurance company contact phone number in the U.S. must be provided</p>

<p>Waiver Deadlines
The waiver request must be submitted by the following dates:</p>

<p>Fall 2009 waiver deadline – September 11, 2009</p>

<p>Spring/Summer 2010 waiver deadline – January 29, 2010</p>

<p>IMPORTANT</p>

<p>Students insured under their parent’s plan, can face the possibility of not being covered in the Los Angeles area if their managed care plan does not provide local coverage. It is best to first check with your carrier if you are waiving the USC insurance plan.</p>
