So........ if we didn't receive the questionnaire, it means that we are either completely in or completely out?

That's the sorry situation.

Those who received the questionaire, mind sharing your stats (GPA and SAT) ?

I think I recieved a different questionnaire. Mine is concerning the mental condition I mentioned in one of my essays, and although I saw the form on MyBerkeleyApp a few days ago, I recieved an email about it today, the 17th. Anyone else in the same boat? I don't know if this means the same thing as the other questionnaire.

I received the e-mail for Berkeley's supplemental questionnaire on Thursday the 17th. That would mean that the questionnaire was due on Sunday--10 calendar days after the e-mail request. Since yesterday was a holiday, theoretically, Berkeley admissions would be seeing my questionnaire information (that I sent on Friday) today. Now here's my problem: I checked the box that said "I intend to send a recommendation." I sent the link to my teacher via e-mail unsuccessfully on Friday night, then I went out of town. This morning she told me she never got my e-mail. She is going to send it within the next hour or so. Will they still accept it?

yes. the recommendation letter is due within 10 BUSINESS DAYS. the supplemental questionnare, on the other hand, is due within 10 CALENDAR DAYS.

This is right off the rec letter form:

"Applicants have been instructed that this letter must be returned within 10 business days of our request."
