<p>I am just looking over this Appointment package and getting ready to send it off to Annapolis, but I have a few questions. I don't have any tattoos of piercings, but I have permanent retainers on my teeth. Am I supposed to document said retainers on the Body Alteration form, because the paper mentions dental ornamentation?</p>

<p>Also, how many places am I really supposed to get a Police Record check from? I only had one minor traffic incident in the jurisdiction of my home area. Should I also get one from the jurisdiction of my school (since it is not the same one)? If so, how would I do so, becaue my school does not have an official 'law enforcement office'?</p>

<p>Yes I believe you document the retainers as dental ornamentation. Also, which ever county you live in which i guess is also the county that gave you the traffic violation or whatever so I believe that is where you would do that.</p>

<p>I'd be very surprised if that's considered ornamental, and that this is the appropriate place for reporting that. My guess is with the dental x-rays.</p>
