<p>Hi, I'm a transfer student and I know we're supposed to mail proof that we got the bacterial meningitis vaccine...but I don't know where we're supposed to mail it to. Could anyone tell me what I need to do? Or where to mail it? and what to mail?

<p>We’re supposed to send in a form and our records, but I cannot find that form. We can either mail, e-mail, or fax them.</p>
<p>Basically, how others have found the form, is:</p>
<p>If you login to your application status check (utdirect) page, you’ll see it has a new dashboard. Click the Housing button. Then you’ll be on a page that says ‘you have accepted your housing contract’ and next to that statement is a link that says “you may check your housing status online”. Click that link. Then, at the very bottom of the page is a link to a pdf of the vaccination form.</p>
