Meningitis shot

<p>I have the shot, but I did not know about showing it during orientation. Is there a way I can send a copy of my shot record, or something, to the school to show I have my shots?</p>

<p>You can mail in your vaccination record to housing services or fax it in. I faxed it and got confirmation within a few hours</p>

<p>Last year I scanned copies of my completed vaccination form along with my record and emailed them to <a href=“mailto:housing@austin.utexas.edu”>housing@austin.utexas.edu</a>. I received confirmation within a couple days.</p>

<p>I got the Meningococcal shot. Is it the same thing?</p>

<p>So, just to make sure, I just need to send a copy of my vaccination record and that’s it?</p>

<p>Edit: Also, I never got anything that told me I need to submit my meningitis records, just to update my medical records (doctor, insurance, etc.).</p>

<p>If you login to your application status check (utdirect) page, you’ll see it has a new dashboard. Click the Housing button. Then you’ll be on a page that says ‘you have accepted your housing contract’ and next to that statement is a link that says “you may check your housing status online”. Click that link. Then, at the very bottom of the page is a link to a pdf of the vaccination form.</p>

<p>Huh, I don’t have that button. I just have the printable terms and conditions, but nothing for vaccinations. Even my girlfriend tried talking me through it earlier, and where she said it would be, it was not there. I’m a transfer, so idk if that has anything to do with it.</p>

<p>So at the bottom of you housing status check page, you don’t have a button that says “view printable vaccination form”? </p>

<p>Maybe it is different for transfers. My son said he was prompted on his main status page after he was accepted several times, and on his housing page, and he rec’d several emails until he finally faxed the forms to housing. I would contact Housing on Monday and get them to send you the link to the forms. If I understand correctly they want to establish your vaccine occurred at least 10 days before move-in and the sooner you do it the less likely you’ll encounter complications during check in. </p>

<p>btw…wish the pdf had a link so I could just put it here. But when you click the “view printable vaccination form” button, it generates a customized pdf with the student name, UTEID, address, social, etc… all filled in on the form. So it’s not linkable.</p>

<p>Hey darkryder, i called dhfs today and they said that the shot wasn’t required of me or the forms or anything. :slight_smile: The lady on the phone said that because we’re transfers it’s not required.</p>

I’ll try to call again later though.</p>

<p>I also talked with DHFS and they said it wasn’t required of me, so I’m not going to worry about it. However I do have proof, just in case.</p>
