Where's the meningitis form? Has your housing contract been confirmed?

<p>Whew, UT Housing has confirmed my housing contract is there. What a wild week we had last week. I hope everyone's is showing completed and we can all give a sigh of relief. </p>

<p>Also from Housing - "Please send the Meningococcal Meningitis Vaccination Requirement Form plus supporting documentation to the Division of Housing and Food Service (DHFS) as soon as you have accepted your housing contract." </p>

<p>I can't fine the form online. Will they email it or where can we find the form online? It says it must be dated at least 10 days prior to check-in but how early (like now?) can we get the shot? </p>
