The TX legislature requires meningitis vaccine - where's the UT form?

<p>I asked this a while back but got no responses so here it is again.</p>

<p>New students living in UT dorms as of Jan. 1st MUST have proof they have had the meningitis vaccine at least 10 days prior to moving in. I've read and signed off on the "what is meningitis" thing online but I still can't find the form your doctor has to sign to verify you got the shot. Where is the form?</p>

<p>Did you find the form? My recollection is that it was an NCR form so you needed a hard paper copy.</p>
