Most schools only require 1 teacher /1 counselor recommendation.
Can I send in the 1 teacher 1 counseor and 1 additional mentor recommendation even if there is no Evaluation Form or Requirement for it?

Unless the school specifically says you may not send in any other information (and some do), you could send in one more recommendation, especially if it speaks to aspects of yourself that wouldn't be covered by the required recs. Your mentor could write a letter on his personal letterhead or stationary- it shouldn't be on the forms that are specifically for the teachers and counselors. If the colleges don't want to read it, they won't, but I don't think one more rec would hurt you.
My son sent an extra rec to his colleges in this manner. I think it's fairly common.

My son also sent an extra rec. to fill in an area his other two recs didn't address. I think it's fine.

At one college info session we went to the admissions representatives were sort of laughing about applicants who send in lots of letters. They told a story about the applicant who sent in the record number: 15! Then one prospective student in the group asked them, "Did he get in?" And neither admissions person could even remember.

Anyway, I think one extra is okay. Maybe it will be useful, maybe it won't. Just make sure, like cronie says, that the school doesn't specifically say "no extra material" with your application.

Ya I'd say sending one more definitely wouldn't hurt. Maybe it won't help either but I would still do it.
